Home Assistant corrects a November beta problem that made OpenRouter conversation agents perform each requested action twice. Asking to add milk to a shopping list could add two entries, while toggling a device could immediately undo the first toggle.
The integration now passes each action on only once when it reads the streamed response. This also avoids duplicate action identifiers that caused Anthropic and Upstage to reject the next request, preventing Claude models from using tools.
